Users being automatically unsubscribed

I've been using phplist for a few years now and recently I've had a few email addresses that I use unsubscribed automatically without my authorization. Another member reported being unsubscribed the same way.
I was just minding my business and one day one of my email accounts on the list received a message saying Goodbye from our Newsletter, sorry to see you go.
How can I prevent this from happening. I could be losing valuable clients by the day and only just finding out about it.
I'm on version 2.10.12
Is this a known bug? How can it be prevented?

Look at the user history, view a user's details then the history link, to see from which IP address the unsubscribe request came. That might help to explain what is happening.

Note that there is no authorisation for unsubscribing, as a user needs to be allowed to do that.

Hello,
The IP is listed as 98.165.106.15
Someone in atlanta apparently. It just happened to one of my admin email addresses. Am I being hacked? Is there a way to prevent this from happening?

I wonder if the intended subscriber(s) have forwarded their copy to someone who doesn't wish to receive the message and they have clicked unsubscribe, this will unsubscribe the original or so I've been led to believe.

It may be worth your time to upgrade to 2.10.19 as 2.10.12 has seen a number of security fixes

Latest phpList version is now 3.0.12 (3 February 2015) and requires a minimum of MySQL 5.0 and PHP 5.3.x
